Warranty Registration

Allard grants a manufacturer’s warranty for defects in material and workmanship in addition to our 30-day Patient Satisfaction Guarantee for all Allard AFOs from date of initial fitting. Warranty registration must be submitted within 10 days of initial fitting to validate warranty.

EASY - Complete at time of fitting. If BILATERAL, it is okay to use the same form for both orthoses.
